Almost everything on a first-visit list. Along Spaccanapoli: Gesu Nuovo, Santa Chiara and its majolica cloister, San Domenico Maggiore and, a lane away, the Cappella Sansevero and the Veiled Christ. Along Via dei Tribunali: the pizzerias, Napoli Sotterranea at Piazza San Gaetano and San Gregorio Armeno, the street of nativity workshops. At the east end, the Duomo and Pio Monte della Misericordia with its Caravaggio. At the north-west corner, the MANN.

For food, the famous names are here or close: Sorbillo and Di Matteo on Via dei Tribunali, and Scaturchio on Piazza San Domenico. Our decumani walk links them in one loop.

Two lessons. Prices in the old town swing hard with demand: Piazza Bellini's standard room cost three times as much on October 14 as on November 11. And the old town is mostly small hotels and B&Bs, which sell out; Costantinopoli 104 had no rooms at all on our first date. The city tax, since May 1, 2026, is €6 per person per night at four-star hotels, €5.50 at three-star hotels and B&Bs, and €6 in short-let apartments.

This is a lived-in neighborhood, not a museum quarter. It has two universities, bars around Piazza Bellini and Piazza San Domenico, and scooters in the lanes until late. The city's own traffic rules reflect the nightlife: the Via del Sole and Mezzocannone gates stay active until 2 a.m. on Friday, Saturday and Sunday nights. If you sleep lightly, ask for a room facing a courtyard or on a high floor, and avoid rooms over Piazza Bellini at weekends.

Line 1 of the metro skirts the old town with four stations: Dante and Museo on the west, Universita on the south and Duomo, opened in August 2021, on the east. Line 2 stops at Piazza Cavour and Montesanto. By OpenStreetMap foot routing, Piazza San Domenico Maggiore is 24 minutes (1.8 km) from Piazza del Plebiscito, 20 minutes (1.5 km) from the ferries at Molo Beverello and 24 minutes (1.8 km) from Napoli Centrale. Piazza Bellini to the MANN is 9 minutes.

Most of the streets are cobbled lava stone, uneven and without pavements. Bring shoes with grip, and think twice about wheeled luggage on the lanes; a taxi to the nearest corner is worth it. See getting around Naples for tickets.

The old town is a ZTL, a limited-traffic zone watched by cameras. The city lists these gates and hours:

• Via del Sole and Mezzocannone: Monday to Thursday 9 a.m. to 10 p.m.; Friday, Saturday, Sunday and holidays 9 a.m. to 2 a.m. the next day.
• Via Miroballo, Via Duomo (at Piazza Museo Filangieri) and Via Santa Sofia: every day, 9 a.m. to 5 p.m.
• Piazza del Gesu: pedestrian, 24 hours a day. Via dei Tribunali is also a pedestrian area.

Hotel guests' vehicles are an authorized category, but only if the hotel registers your plate before you pass a gate. If you are renting a car for the coast, pick it up on the way out of town, not before your city days.

The realistic risks here are the ones the UK government lists for Italian city centres: pickpocketing and bag-snatching in crowds, including from scooters. Keep your phone in your hand on the wall side of the lane, carry your bag across your body, and do not hang it on a chair at a pavement table. The lanes are busy with residents and students into the evening. Stay here on a first trip of two to four days, if you want to walk to the sights and eat pizza without a taxi, and if you like a city with its sleeves rolled up. Stay elsewhere if you need quiet after midnight, a view of the bay, step-free streets or a car at the door. For those, look at the Lungomare or Chiaia.
